Once you have scheduled your appointment with Dr. Clark, please ensure that you bring the following items to your first exam
- Insurance Card
- List of Current Medications
- Current Eyeglasses and/or Contact Lenses
Our comprehensive eye exam consists of over 30 different tests and takes about 30 minutes. At the end, Dr. Clark will review all exam findings and answer any questions. We believe that better communication leads to better health outcomes, so ask anything & everything you ever wanted!

Don't have insurance? No problem! We offer a comprehensive eye exam with widefield retinal photography, a glasses prescription, and a contact lens prescription for $150.
